Event Participation Authorization

I acknowledge that I have been informed that, as a member of the Youth Group of [Temple of Christ], [Lagrange], [Georgia], my child will participate in activities during the academic year [January 2007 – December 2007] which carry a degree of risk. The activities include stepping, spiritual vibe to gospel or inspirational music, acting, prop-making, sewing, modeling, singing, rapping, field trips, and other activities which the church may offer, such as [picnics and occasional meals]

I consent for my child to participate in these activities.

I also state that my child is in good physical condition and has the necessary skills to participate safely in these activities.

You may rely on the above consent until I notify you in writing of any changes.
